Day-to-day management of patients with complex chronic illnesses who demonstrate high utilization of health care resources.
Services:
- Monitor health status through the development of long-term relationships
- Assist with medication management, patient and family education
- Access community and home care services that benefit patients
- Discuss and help individuals make choices regarding the end stages of life
- Prevent duplication of services
- Evaluate proper health resources and make referrals
